Flo Med Spa & Wellness also provides core medical services designed to keep you healthy, certified, and cared for — whether you’re managing chronic health needs or simply maintaining your wellness routine.
CONCIERGE MEDICINE
Our Concierge Medicine program takes a modern, personalized approach to healthcare. Instead of rushed visits and long waits, you receive direct access to your provider, longer appointments, and a custom health plan tailored to your lifestyle.
Concierge Medicine Tiers:
TIER I- Individual Membership- with no past medical history
· 1 visit each month
· Annual well check visit- DOES NOT INCLUDE LABS
TIER II- Individual Membership- up to 2 medical problems/diagnosis
· 2 visits each month
· Annual well check visit
· Labs- routine labs every 6 months
TIER III- Individual membership-up to 3 medical problems/diagnosis
· 3 visits each month
· Annual well check
· Labs- routine labs every 6 months
TIER IV-Individual membership, more than 4 medical problems/diagnoses
· 4 visits each month
· Annual well check
· Labs- routine labs every 6 months
TIER V- Family membership- 2 parents and 1 child under 18
· 4 visits each month (one for each member, visits not to be shared)
· Annual well check for each member
· Labs- routine labs every 6 months

Term/Membership Fee: Flo Medspa and Wellness in the amount determined by the appropriate TIER above. The monthly fee will be charged to the member’s credit card on file. Failure to pay the monthly membership fee on time will result in automatic termination of the membership and loss of the program's benefits. Renewing membership after termination will incur a $50 reapplication fee.
Termination of Membership:
Member shall have the ability to terminate membership at any time. However, the member will not be entitled to a refund. Flo Medspa and Wellness, PLLC reserves the right to cancel any membership for cause, including but not limited to non-payment, non-compliance, threats to staff/provider/patients (verbal or physical), or damage to property. Termination of membership will incur a $50 renewal fee if the member is allowed to restart the program at a later date determined by the provider/office manager.

The monthly membership fee covers only the services listed. If the healthcare services excluded from the monthly membership fees are provided, the member will be financially responsible for these charges at the time of service. Services excluded can change without notice by the provider.
